CRC membership account requests are now 100% online! Use the below forms to make a membership request. We will reach out if we need additional information. Once your request has been processed, you will receive an emailed copy of the form or a confirmation email from our team.
New Memberships
Make Changes to an Existing Membership
- CRC Membership Change Request - Add or remove someone from an existing membership (upgrade/downgrade) or change bank draft
- CRC Membership Hold Request - Place a hold on your membership free of charge through June 2021. Members currently on hold through January will be extended through June 2021.

Financial Assistance
Due to safety considerations, membership transactions will not occur at the CRC and must be done online. If you would like assistance with your account or an online request, please email membership@mhcrc.com or call 408-782-2128 Monday-Friday between 9am and 4pm.
